You may be mailed some of these forms before your first session. You may also be asked to fill additional forms out from time to time by your therapist. These forms are specifically intended for use by individuals receiving services at the Center for Psychological Services and NOT for Forms for the Cognitive Assessment Program.
The Center does not accept third party payment nor do providers complete pre-authroization paperwork for insurance reimbursement. You will receive an itemized receipt for services that you may submit for insurance or other healthcare reimbursement. All fees are due at the time service is offered. Center requires cash, credit card, or check at time of service.
